Which is the place of burch surgery by laparoscopy ? This doubt is more present than ever. In terms of results after at least 48 months of follow-up the retropubic suspensions and slings were more efficient than the transvaginal suspension and the former restorations, according to conclusions of the American Agreement of surgeries for stress Urinary Incontinence in women: These techniques, never the less, and, mainly the slings, show more complications according to the conclusions of the same agreement. At the American Congres in 1999 the results shown of the Burch surgery by videolaparoscopy with more than 2 years of follow-up were very compromising, because only half of the patients remained continent. The question that arises as na immediate consequence is: Why is the Burch surgery efficient by classic laparotomy and inefficient by laparoscopy? The videolaparoscopy is nothing more than a new form of doing traditional operations using the video resources, amplifying images and insuflation of gas in the intraperitoneal space. We changed the form of the scissors of the suture device and tweezers to adapt them to the new situation, but we used the same principle of dissection, hemostasis and tissue synthesis. It is true that we added intra operative complications due to the punctures and the pneumoperitoneum, but on the other hand we diminished recovery and other post operative complications. Maybe gas or even abdominal pressure have some negative influence in the results. The fact is that the only explanation that seems logic at this moment is that the failure in the results could be due to mistakes in the execution of surgical techniques such as the application of one only stitch in the vaginal wall in the urethrovesical junction that many practice, instead of two or three stitches. As follows we show some important aspects of the surgical technique. A dissection must be made, rigorously respecting the anatomy, identifying the bladder neck, vaginal wall and the Cooper' s ligament. Three punctures are enough. In the hollow of umbilicus a trocar is applied, the lateral punctures must be made in the iliac fossa, right and left, a little above the ridges iliacs anterosuperior. Lower punctures make it difficult or even impossible to dissect. Rigorous hemostasis make easier the visual operative field and dissection. The Retzius space is better dissected filling the balloon with 200 to 300ml of serum but for the dissection of the bladder neck and vaginal wall the bladder must be emptied to facilitate dissection and to avoid vesical lesion. Initially we applied one stitch on each side, but for safety sake, we recommend applying at least 2 stitches on each side of the vaginal wall at the level of the bladder neck. WHO MAY BENEFIT? It is evident that thre is no indication of applying Burch's colposuspensão technique if there is na intrinsic lesion because there are other techniques at hand that are more efficient. But if there is no intrinsic lesion of the sphincter, mainly in younger patients with stress Incontinence or those that are going to have a laparoscopica operation of a different nature as colecistectomias, hernia de hiato, hernia inguinal, nefcrectomy, laqueadura, ovary cyst, and many others, surgery by video laparoscopy is the most natural method. It seems tome premature to discard this surgical resource at the moment, because there are conflicting results in literature relative to the results of Burch surgery by videolaparoscopy.
