One of the problems that we must face with relative frequency is the hypercorrection or obstruction of the urethra after the attempt of surgical correction of stress urinary incontinence (SUI) in women. This may happen with all surgical techniques, and frequently is related with the degree of tension (excessive) in the sutures of sustentation of such surgeries. Today we accept that in general, it is better not to tighten these sutures too much, specially in the suspensions with needles and in the slings. When it refers to correct hypermobility, this is a rule, but it is not so simple when the problem is mainly in urethral resistance, specially in severe SUI cases type III, since in these cases it is normally necessary to compress the urethra to obtain continence. How to treat postoperative obstruction? Lets schematize our conduct beginning with prevention measures: * Not tightening the sutures after reaching the tension level that pulls the tissues and/or the suture threads allowing that a cystoscope sheath or a urethral bougies straight may be lowered to enter into the bladder though the urethra. * Setting a retropubic catheter to treat with comfort immediate postoperative retention, frequent till the first postoperative week. This must not be take out before you get the least residue, 50ml, or inferior to 10% of the vesical capacity, keeping the catheter closed for 24/48 hours for the last residual control before removing it. * It is important to determine that a percentage of the patients will reach this objective after 3 or 4 weeks of the postoperative. * If in 15 days of the postoperative the residual does not lessen, we indicate corticoids +. Sodium Diclofenac by intra muscular injection. * If in 15 days spontaneous urination is not recovered, or if in 30 days a high postvoid residual persists, we make a urodinamic study (video-pressure-flux). * Obviously, this study may be done any other time different than the one proposed, but its usefulness is related with the measures that we will take in relation to the time of the postoperative that has passed. * This study may result in obstruction or hipoactive bladder contraction (impaired destrusor contraction). * If the obstruction is recent (till 45 days post operative) maneuvres to free may be tried or traditional aproach: - To loosen the retropubic suture and execute a counter-traction, or lower the urethra with the cystoscope sheath and re-tie the suture (looser). - One side of the suspension may loosen, keeping the far side, re-fastening it (with a needle) in firm retropubic tissues. - The sling may be cut in the middle line (transvaginal aproach), without mobilizing it with which you obtain to decompress and at times, to keep the suspension with the finality of maintaining continence. - If the time elapsed is more and the tissues are consolidated (fibrous), Surgical Urethrolysis must be done, (urethral liberation) of all the adherences and once they have reached mobility, a new suspension or support is proceded, in an adequate way, without tension. * The urodinamic diagnosis may be deterioration of the detrusor contraction. This condition is normally pre-existant, and the patient "compensated"with the participation of an increase in abdominal pressure and using the stress urinary incontinence mechanism (urethrovesical hypermobility) to obtain urination, by being corrected by surgery, spontaneous miction is very difficult or impossible. We then must consider the option of intermitant catheterism to maintain continence.
