Overactive Bladder A overactive bladder is a commonly frequent cause of urinary loss and is related to a condition that involves urge-incontinence symptoms, frequency and urgency. It is characterized by involuntary contractions of the detrusor which occur spontaneously or then, provoked by a quick filling of the bladder, posture alterations, coughing, walking or jumping, at the same time that the patient is trying to stop it. The involuntary contractions are always symptomatic, associated to the need of urination, urgency and urge-incontinence. It is important to observe that urinary incontinence is a symptom and not a specific illness. To treat this it is necessary to evaluate the cause of urinary loss. Urge-incontinence, in some accounts, is constituted in the most common type of urinary incontinence, it may reach 65% of the cases, it is more common in women than in men. Some patients with urge-incontinence complaints do not present involuntary contractions during urodynamic studies. When these patients are examined in an out patient way, 40% to 84% present detrusor involuntary contractions. This way, detrusor instability may be a bigger problem in the incontinent population than the conventional studies may show. The prevalence of a overactive bladder may be underestimated because the studies about this are directed to patients with incontinence complaints. Urinary incontinence is associated with a reduction of physical and social activities, with more emotional problems, and the evident reduction of life quality. However, urinary loss is only an aspect of a overactive bladder. The individual effects include loss of self-esteem, reduction of social activities, sex life restriction, neurosis and depression. The shame due to urinary loss and the odor of urine, inhibit physical and social activities, resulting in social isolation, anxiety and depression. Now a day, most cases of urinary incontinence and overactive bladder may be cured, using a combination of treatments. This includes pharmacological treatment, behavior treatment, and physiotherapy treatment of the pelvic floor.
